The student only needs to provide basic information such as:

  • Student Name
  • Registration Number
  • Training Duration
  • Basic Personal Details


Employer’s Responsibility

The remaining evaluation is completed by the employer or training supervisor, including:

  • Performance Assessment
  • Professional Behaviour
  • Learning Ability
  • Work Ethics
  • Overall Remarks
  • Signature and Official Approval


Students should never fill in the employer’s evaluation section themselves.

The report has two sections.

Is an Official Letterhead Required?

No.

ICMAI has already provided a standard Performance Report format.

Students simply need to download the prescribed format, fill in the required student details, get the employer’s evaluation completed, and upload the signed report.

An additional letterhead is generally not required unless the employer wishes to provide separate remarks or supporting comments.

Who Needs to Submit the Performance Report?

This is where most students get confused.

You Need to Submit the Performance Report If:

  • You are submitting your T5 Form now.
  • You are submitting your T5B Form now.
  • Your training completion form has not yet been uploaded on the ICMAI portal.


In simple words, students submitting T5 or T5B after this update should keep the Performance Report ready.

Who Does NOT Need to Submit It?

You generally do not need to submit the Performance Report if:

  • Your T5 Form has already been uploaded.
  • Your T5B Form has already been uploaded.
  • Your form is already available on the ICMAI portal, regardless of whether it is approved or still under processing.


If your training completion form has already been submitted before this change, there is usually no need to upload an additional Performance Report.

Many organizations prefer to avoid additional documentation, and some employers may initially hesitate to complete the Performance Report.
